Assessment and Intake Process

What to Expect: Our compassionate intake coordinators will conduct a confidential assessment to understand your unique situation and needs. This typically takes 45 minutes and can be done by phone or in person.

We’ll Ask About:

  • Current living situation and support system
  • Stage of memory disorder and specific challenges
  • Caregiver stress levels and support needs
  • Previous services utilized
  • Immediate safety concerns


Next Steps:
Based on your assessment, we’ll create a personalized service plan and connect you with appropriate resources within 48 hours.
